Shoulder pain

So my orthopedic doctor put me on duloxetine + vitamin b12 and it helped a lot a with my back pain and knee pain, but apparently showed no affect on my shoulder pain, can somebody tell me if I am suffering from something else particularly related to chronic pain in shoulders.
