Who is behind Mark?

We clearly saw Mr. Milchick wearing a different color shirt shortly after this scene. I doubt its Graner since we saw his brain get turned into mashed potatoes last season lol. I have a feeling that if you have some sort of blunt force trauma to the head, theres no way you can do anything to the brain like getting severed or something, the damage is too much. Right?
